We are currently accepting new patients. You can obtain our new patient forms prior to your visit. Please send your request to firstname.lastname@example.org. Once completed, please FAX the forms to our office.
We request our patients to arrive 30 minutes before their scheduled visit to complete any additional appropriate form for your first visit.
Please bring relevant past medical records (especially pediatric immunizations, diagnostic test results, and the names & contact information of your Primary Care Doctor and any Specialists under whose care you are.
If you are currently taking any medications, supplements, vitamins, or herbals, please bring in their original bottles.

Important clinic FYIs and Policies:
We will not refill prescriptions for controlled substance medications over the phone. If you need a standard Pharmaceutical medication, we can electronically send it to your chosen pharmacy if Dr. Vizinas is your PCP.
- If you need a refill on a supplement, we can be mailed it to your chosen location, picked up in our office, or bought online though a patient portal. Our staff will give you information on how to order online. The best method to get your prescription refilled is to request it during your office visit.
- Forms to be completed (physical, FMLA, disability, worker’s comp among others): We charge a fee for the time involved in filling out these forms. Charges vary depending on the complexity ranging from $35 to $50.
- We require that you indicate a PHARMACY on file, even if you are not on any medications.
- Last Minute cancellations/No Show Visits: Last Minute cancellations will incur a $50 fee. If you are experiencing an emergency that will interfere with the appointment, ALWAYS CALL! To reschedule an appointment, please give our office at least 24 hour notice. There is a patient waiting list and we would like to use the opportunity to provide care for someone else in need.
Full payment is expected at the time of service if insurance doesn’t cover the service provided. Payments can be made by cash, check, and credit card. Payment plans and discounts are available based on personal need. Please discuss your concerns with our Office Manager or Biller before your appointment.
For patients with high deductible and out-of-network benefits, you are responsible for our fees at the time of service. Remember, some of the services may not be covered under your plan. We’ll give you information at the time of your visit, but we cannot guarantee that such testing will be covered through insurance. It is the patient’s responsibility to check for coverage. Our clinic will bill the insurance if we are in-network, but the patient is responsible at the time of service for deductibles, co-pay, co-insurance, and any services previously identified by the insurance carrier as not covered.
Dr. Zdravkov is does not work with insurance. However, his acupuncture service is occasionally covered by some insurance plans. Please check with your insurance carrier.
Dr. Vizinas is currently in network with Blue Cross Blue Shield, Medicare, United Healthcare, Cigna, and Aetna. We recommend you contact your insurance plan prior to the visit to ensure coverage.
